H1N1 Vaccinations Article Date: 09/25/2009
OPNS is providing you with the forms needed to enroll to participate in the H1N1 vaccine administration program. Since this is a public program, distribution will be handled through the Oakland County Department of Health and Human Services. If you wish to participate in vaccine administration, you will need to complete the attached documents return them to the Oakland County (fax number 248 858-0246). You do not need to be in Michigan Care Improvement Registry (MICR) yet but will have to be online when you begin to administer vaccines. Your Vaccine For Children (VFC) Pin is not needed at this time to enroll. If you do not have those numbers leave that blank and it can be filled in later. OPNS has met with the officials and they are familiar with our organization. If you have questions or concerns feel free to contact us and we will be glad to help.
